How Can we Cutomise Your Care ?
The process is fairly comprehensive. To ensure that the level of service you are receiving is good and fulfills your expectations, this is done. Everyone has different demands, thus it is not necessary to use every service if you do not need it. We must make sure that all of your needs are met because you have your own personal goals in life.
Your medical needs will first be ascertained through a process by your registered nurse clinical coordinator. This includes your living situation and personal preferences. If necessary, your loved ones can get involved. If they require specific care, that can also be met. This is all done in coordination with your general practitioner and the expert, according to their recommendations.
Direct communication is used to design and modify a service plan to suit your needs. Of course, you can have a spending limit in mind. Because of our versatility, you can get the services you require either by combining them with the ones you now receive or by acquiring all of them if you do not currently receive any.
After learning about your personal and medical needs, the appropriate personal care assistant or nurse is subsequently allocated to you. They will go above and beyond to see to it that you have all you require.
Your care support is regularly monitored by a nurse case worker. After all, the primary concerns of the personal caregiver are your happiness and well-being. As your initial clinical coordinator, the caseworker will get to know you well and recognize your unique preferences. We understand the significance of continuity in care, which must also be as unique as possible.
Because you are not constrained by a contract, you have far more peace of mind. We will regularly ask for feedback from you, your caregivers, and your loved ones and make adjustments to ensure that the care you get is of the highest caliber.
